Case Management
| Whether it
is poor credit, a nonexistent rental record or a history of
incarceration, destitute, AIDS-impacted persons often face
what seem like insurmountable barriers to residential stability. |
Without
access to appropriate and affordable housing, indigent persons
living with HIV/AIDS are at constant risk for homelessness and
poor health. Through our Housing Case Management Program,
clients are able to overcome many of the barriers to finding,
and holding on to, a stable place to live.
The program links clients
with on-staff Housing Case Managers, who work proactively with
the client to formulate housing plans, build resources, access
appropriate housing, navigate the search process, apply for housing
assistance and make the actual move into housing. A significant
issue is the need for affordable housing, a countywide phenomenon
among the poor that is even more acutely felt by people who are
chronically ill with HIV/AIDS.
In addition to helping clients move from wait lists to actual
places to live, our Housing Case Managers educate clients about
tenant rights and responsibilities, neighborhood amenities and
services, and act as liaison between the client, property owner
and collaborating agencies.

exit planning and support services for clients leaving The
Serra Project's Group Homes and Permanent Supportive Housing
(CHOISS) programs

both homeless and at-risk individuals and families living
with HIV/AIDS and a secondary diagnosis
